This item can be found inside the Four Horsemen Chest after defeating the Four Horsemen as well as a small chance from dropping off of Gluth in the 25-man version of Naxxramas.

Trivia

The handle of the blade seems almost fully identical to the one of "The Armageddon Blade" from the First Darksiders game, published by THQ and developed by Vigil Games. However, the blade of Armageddon does not match "The Armageddon Blade"'s.
