This is one of the eight available Darkmoon Cards.
It has been known to be a "party saver" on a few occasions. This trinket becomes more useful when soloing (so you don't have to run back when you die). In 5 manning it serves as last ditch wipe protection. It won't help you in 40mans, unless you're a backup healer that tends to die a lot. On world bosses where a death means trouble for a raid (Lord Kazzak, etc), it probably won't be any use to you. However, it doesn't automatically resurrect you, instead it works as a soulstone and gives you a "Twisting Nether" option, along with "Release Spirit."
To obtain the Twisting Nether card you make complete the Portals Deck. The Ace of Portals drops from Darkmaster Gandling from Scholomance.
With the advent of The Arena, the Twisting Nether has seen a surge in popularity. While the chance of proccing the effect is low, and the benefit of the proc quite random, the chance to resurrect in an arena battle can result in a victory that might otherwise have been a loss.
Darkmoon Card
Darkmoon Cards are obtained by completing a set of eight cards (The relevant Ace plus 7 general cards, numbered 2 of X deck and up to 8 of X deck - like, for example, Four of Beasts). After obtaining the eight cards the player can create a Deck item by clicking and activating the Ace card. All the individual cards and the deck itself are BoE items and can be bought in the Auction House.
Each Ace drops from a specific elite boss somewhere in the game, which usually have an estimated (unverified) 10% drop chance (since 1.10 patch). The general cards are world drops from ANY general humanoid level 51 and above, including elites. There are no specific place or mob they can drop from. The drop rate is slightly better for elite dungeon bosses.
Once you have the deck item, you can turn it in the Darkmoon Faire for your final Epic trinket.
Note
This trinket does now work with Spirit of Redemption Priest talent.
