Death's Embrace is a quest reward from the quest 
[60R] Death's Embrace which starts with Zanza the Restless in Zul'Gurub. To complete the quest, you must turn the following items to Zanza the Restless:

The quest to obtain this is repeatable so you can get multiple enchants.
Oddly, the +28AP from this enchant does not apply to Ranged Attack Power. It only adds to melee attack power.
