Eye of the Stalker is a rare drop from The Black Stalker, the final boss of The Underbog wing of Coilfang Reservoir. It only drops in Heroic mode.
Notes
It is identical to Ring of Ghoulish Delight. Despite the difference in name and icon, though, you cannot have both of them at the same time.
