This item is a rare drop from Headless Horseman. The horseman is found in the graveyard wing of Scarlet Monastery during the Hallow's End seasonal event.
Notes
It is BoU, and has a 14 day duration from when first used.
Do not fly into Zangarmarsh while using the [Flying Broom]. Doing so will dismount you in midair and cause you to fall. Attempting to use the [Flying Broom] in Zangarmarsh will give you the message The weather isn't right for that. However, the [Swift Flying Broom] will work fine.
The item description indicates that it is one-use, but this is not correct.
