Item given by Altruis the Sufferer as a part of the quest  [70] Against All Odds. It is used to summon the demon Xeleth from the bottom of the lake where he slumbers.

The location where Xeleth appears is very close to a former gateway used by Magtheridon to bring legion reinforcement to outland, this may explain the presence of the demon in such an unusual location
