Iso'rath is a massive servant of the Old Gods living in the Maw of Madness^{[48.3, 14.4]} in the Twilight Highlands. The Earthen Ring is launching an attack on it from the nearby Ring of the Elements, but are fighting a losing battle as they are thrown inside its stomach by its tentacles. Players and the Earthen Ring alike endure a series of horrific tribulations, ranging from being slowly digested by its stomach acid to given a false vision about failing to save the world from Deathwing.
