The "hitbox" is a radius that is added to attack ranges to account for the attacker and target's sizes, since the game treats each unit as a single point when checking range. Players and most mobs have a 5-yard hitbox; bosses often have larger hitboxes. Since melee attacks have a nominal range of zero, you can attack a target in melee only if your center point is inside its hitbox.

Area of effect checks also add the hitbox; the mob ability Swell demonstrates this, since it has a zero-yard range but still hits players.
