On release of Patch 3.2.2, the "Onyxia's Lair" Raid achievement was converted to this Feat of Strength.

This Feet of Strength could be gained after 3.2.2 by obtaining an item from Nefarion in Blackwing Lair. This was lasted tested worked before 3.3.0. It is unknown if it still works.
