Scarlet Ghouls are created from Scarlet Miners in the Havenshire Mine as a result of using the  [Gift of the Harvester] while on the quest  [55] The Gift That Keeps On Giving.

In phase two, Scarlet Ghouls are created by Acherus Necromancers on the beach at Light's Point from the corpses of fallen Scarlet Fleet Defenders.
