This item is a reward from the Tranquillien quests Rotting Hearts and More Rotting Hearts which start with Magistrix Aminel in Tranquillien in Ghostlands. To complete these quests, you must turn the following items to Magistrix Aminel:

The Rotting Hearts needed to complete the quests to acquire this item are random drops off nearby ghoul mobs in the Dead Scar, and not actual quest items. As such, you can amass them without having the quest logged, or maybe even buy them from an Auction House.
