Shadra <The Venom Queen> (aka Elortha no Shadra) is a giant level 55 elite spider that is worshipped by the Witherbark trolls in the Hinterlands. The trolls use her venom as a powerful poison for their weapons. She is summoned by using [Gadrin's Parchment] at her altar in Shadra'Alor.
As of patch 2.3, she is no longer a giant, being around as large as the surrounding regular spiders. She is however as powerful as before, so don't get careless. She can swim, so don't try it. The reasons for her no longer being a giant have not been explained. It may be because of mob and pet size normalization.
