[Skullsplitter Fetishes] drop from Skullsplitter Witch Doctors and Skullsplitter Mystics. These trolls can be found in the three central troll ruin sites of Stranglethorn Vale (Ziata'jai Ruins, Ruins of Zul'Mamwe, and Balia'mah Ruins).
