This item can be found extremely rarely inside the [Bag of Fishing Treasures] rewarded from completing Marcia Chase's daily fishing quests in Dalaran.
It can also (again, rarely) be found inside the Tiny Titanium Lockbox, another reward from the Bag of Fishing Treasures.
